Remote chronic care support for blood pressure and cardiometabolic risk.
Cardiowell 360 is a remote-only clinical services organization providing chronic care support through advanced monitoring and care-management programs. We partner with providers and care teams to support RPM, RTM, CCM, PCM, APCM, and CPT 99091 data review and interpretation.
Two ways to access specialized blood pressure support in Colorado.
Cardiowell 360 now offers ACCESS services for eligible Original Medicare patients and a personalized private pay / non-insurance / non-insurance hypertension program for Colorado private pay patients.
For eligible Original Medicare patients
Remote blood pressure support through Cardiowell 360’s participation in the CMS ACCESS Model. Designed for eligible people with Original Medicare and high blood pressure who want monitoring, engagement, and clinician-guided support between visits.
Personalized hypertension care for $100/month
A direct-to-consumer, private pay / non-insurance program for Colorado patients who want specialized blood pressure care without waiting for insurance approval.
Cardiowell 360 is preparing for Medicare patient and clinician referrals through ACCESS.
Starting in July, Cardiowell 360 expects to be listed for ACCESS referrals. Eligible people with Original Medicare and high blood pressure may be able to receive remote blood pressure support through our clinician-guided program.
