Private Pay Hypertension Program

Personalized hypertension care from $50/month

A direct-to-consumer, private pay / non-insurance program for Colorado patients who want specialized blood pressure care without waiting for insurance approval. Your first month is $150 and includes the connected blood pressure device and app; after that it is $50/month, and you can stop and start the monthly service at any time.

Cardiowell 360 has been accepted to move forward in the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model process. The ACCESS Model tests outcome-aligned payments in Original Medicare for technology-supported chronic care services, including high blood pressure.
