Remote Only Chronic Care Management Services
Cardiowell 360’s services are limited to remote monitoring and remote care management. The organization is not a general primary care practice, urgent care center, emergency care provider, or walk-in clinic.
Included scope
Remote monitoring, care management, patient outreach, medication-adherence support, trend analysis, early risk detection, provider communication, reporting, education, and behavioral support within defined programs.
Excluded scope
General primary care, in-person care, emergency care, urgent care, preventive visits, physical exams, procedures, walk-in visits, and broad management outside defined remote care programs.
Remote-only, supervised, and program-defined.
Physician supervision
Programs are supervised by licensed physicians and delivered by appropriate clinical and support staff.
Payer rules
Programs are structured around CMS and payer rules for RPM, CCM, RTM, PCM, APCM, and related services.
Non-clinical support location
The Device Support Center is for device setup, technical support, education, admin, logistics, and equipment management only.
