Remote blood pressure support for eligible Colorado patients with Original Medicare.

Cardiowell 360 is formally part of the CMS ACCESS Model and is preparing to support eligible Original Medicare patients in Colorado through technology-supported chronic care focused on blood pressure and cardiometabolic risk.

Colorado only: Cardiowell 360 ACCESS services are currently available only to eligible patients located in Colorado.
Starting July

Cardiowell 360 will be listed for ACCESS patient and clinician referrals.

The ACCESS Model expands access to technology-supported chronic care options in Original Medicare and tests outcome-aligned payments tied to measurable health improvement.

Who may be eligible

The program is intended for patients with Original Medicare and qualifying chronic conditions.

Original Medicare

You generally need Original Medicare Part A and Part B. Medicare Advantage eligibility may differ.

High blood pressure

You have been told you have hypertension or you are working with a clinician to manage blood pressure.

Remote participation

You are willing to participate in remote monitoring and communication with the Cardiowell 360 team.

Our team reviews whether the ACCESS program appears to be a fit and may contact you for additional information.

If eligible, you choose whether to enroll. Your usual Medicare benefits and freedom to choose providers are not replaced.

Cardiowell 360 is not an emergency service, urgent care center, walk-in clinic, or replacement for emergency care.
