What is the CMS ACCESS Model?
ACCESS stands for Advancing Chronic Care with Effective, Scalable Solutions. It is a CMS Innovation Center model that tests outcome-aligned payments in Original Medicare to expand access to technology-supported chronic care options.
Who can use ACCESS services?
ACCESS is intended for people with Original Medicare who have qualifying chronic conditions in one of the model’s clinical tracks. Patients may enroll directly with an ACCESS organization or be referred by a clinician.
Does this replace my doctor?
No. Cardiowell 360 is designed to complement your existing clinician or care team. It is not a replacement for primary care, urgent care, emergency care, or regular medical visits.
Is this for Medicare Advantage?
The CMS ACCESS Model is described as an Original Medicare model. If you have Medicare Advantage, submit the intake form or contact us and we can help determine whether there may be another appropriate program.
What condition does Cardiowell 360 focus on?
Cardiowell 360’s ACCESS preparation is focused on high blood pressure and cardiometabolic risk, using remote monitoring, education, patient engagement, and clinician-guided support.
Will I receive a blood pressure monitor?
Program resources may vary based on eligibility, program requirements, and available workflows. If a device is needed, Cardiowell 360 will explain the next steps during enrollment.
Does Cardiowell 360 prescribe medication?
Medication workflows depend on program structure, state requirements, clinical judgment, and the patient’s care team. Cardiowell 360 is designed to support monitoring and co-management, not replace the patient’s treating clinician.
What happens if my blood pressure is very high?
Cardiowell 360 may use escalation workflows for concerning readings or trends. However, the program is not emergency care. If you have severe symptoms or think you may be having an emergency, call 911.
How do I get started?
Patients can complete the eligibility form. Clinicians can use the referral form. A Cardiowell 360 team member will review the information and contact you about next steps.
