Cardiowell 360 selected to take part in the CMS ACCESS Model process.

Cardiowell 360 has been accepted to move forward in the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model process.

ACCESS Model

A strong fit for remote, outcome-focused chronic care.

The ACCESS Model tests an outcome-aligned payment approach in Original Medicare to expand access to technology-supported chronic care options, including high blood pressure.

For Cardiowell 360, this is significant because the model aligns with the organization’s focus on clinician-guided, technology-supported chronic care, remote monitoring, patient engagement, and measurable blood pressure improvement.

Important note: Acceptance of an application does not by itself mean final enrollment or participation. Accepted applicants must complete additional participation steps, including signing the Participation Agreement when instructed.
